45+ Health Assessments

The 45+ health check is available to people between 45 and 49 years of age who are at risk of developing a chronic disease.

The health check is available to people between 45 and 49 years of age who are at risk of developing a chronic disease. The aim of the 45+ Health Assessment is to be able to indentify those at risk of developing chronic disease and to put into action preventative measures to assist in the prevention of the diseases.

The health check involves a 30 minute consultation with a Practice Nurse who completes relevant examinations and investigations based on information from the patient’s medical, social and family history. The patient is provided with advice and information and where appropriate, this includes advice on strategies to achieve lifestyle and behaviour changes. The patient is then seen by his/her usual doctor who will discuss any issues that have been identified by the nurse.
