Diabetes Risk Evaluation

Eligible patients must be aged 40- 49 years (inclusive) and at high risk of developing type 2 diabetes as determined by the patient filling out a diabetes risk assessment tool. Patients can ask their GP or Reception staff for this form at anytime.

The aim of the Type 2 Diabetes Risk Evaluation is to identify patients who are between the age of 40-49 years (inclusive) who are at high risk of developing type 2 diabetes. It enables Gp's to review patients risk factors and instigate early interventions such as lifestyle modification programs to assist with the prevention of type 2 diabetes. 

It involves the patients filling out a short questionnaire which is reviewed by their doctor during a consultation.
