Team Care Arrangements (TCA)

A TCA enables a doctor to coordinate the preparation of a Team Care Arrangement for a patient who has a chronic or terminal medical condition and also requires ongoing care from a multidisciplinary team of at least three health or care providers.

It involves the Doctor collaborating with the other participating providers on required treatment/services, agreeing the arrangements with the patient and documenting a plan. Paperwork is sent to the patients' other care providers such as a physiotherapist, podiatrist, dietician and specialists detailing what type of care the doctor requires them to do to enable the patient to meet their healthcare goals. These plans are generally reviewed 6 monthly or annually.
