Government Funded Health Assessments

Providing ongoing care and support for someone who is living with a mental illness can involve many different support organisations. These may include psychologists, GP’s, psychiatrists, nurses or mental health workers.

Mental Health care plans are for people with a mental illness. A Doctor will use a plan to help you work out what services you need, set goals and decide on the best treatment options for you. It may also include what to do in a crisis.

A Home Medicine Review is a review of a patients medication by their Pharmacist and Doctor. Patients on 5 or more medications are eligible for this service.

This health assessment is for an older person and is an in-depth assessment of a patient aged 75 years and over.

The Chronic Disease Management Plan enables GPs to manage the health care of patients with chronic or terminal medical conditions.

The 45+ health check is available to people between 45 and 49 years of age who are at risk of developing a chronic disease.

A TCA enables a doctor to coordinate the preparation of a Team Care Arrangement for a patient who has a chronic or terminal medical condition and also requires ongoing care from a multidisciplinary team of at least three health or care providers.
