Diabetes Cycle of Care

The Diabetes Cycle of care aims to encourage GPs to provide effective management of people with established diabetes. A cycle of care sets out the minimum requirements a GP should complete for a patient with diabetes. Good management of diabetes includes foot & eye checks as well as regular Blood & Urine tests.

It involves a patient making an appointment with their doctor for them to start the process.

Team Care Arrangements (TCA)*

A TCA enables a doctor to coordinate the preparation of a Team Care Arrangement for a patient who has a chronic or terminal medical condition and also requires ongoing care from a multidisciplinary team of at least three health or care providers.

Chronic Disease Management Plan (CDMP)*

The Chronic Disease Management Plan enables GPs to manage the health care of patients with chronic or terminal medical conditions.

Diabetes Risk Evaluation

Eligible patients must be aged 40- 49 years (inclusive) and at high risk of developing type 2 diabetes as determined by the patient filling out a diabetes risk assessment tool. Patients can ask their GP or Reception staff for this form at anytime.

Friday: 8.00am – 6.00pm, phones from 8.30am – 5.45pm

Patients can access medical care after normal consultation hours in a number of ways:
