Mental Health Care Plan*
Providing ongoing care and support for someone who is living with a mental illness can involve many different support organisations. These may include psychologists, GP’s, psychiatrists, nurses or mental health workers.
Mental Health Care Plans are for people with a mental illness. A Doctor will use a plan to help you work out what services you need, set goals and decide on the best treatment options for you. It may also include what to do in a crisis.
